Provisional Licence
1st Provisional Licence
Licence application must be accompanied by:- Theory Test Certificate
- Eyesight Report

- The first provisional licence is valid for two years
- The licence holder, with the exception of those who hold provisional licences in Category M, A1, A or W must be accompanied by a qualified driver at all times while driving.
- You may apply for categories EB, C1, C, D1, D, if you have a full B Licence
- You may apply for Categories EC1, EC, ED1, ED, if you have a full licence in the relevant drawing vehicle.
Second Provisional Licence
Licence application must be accompanied by:- Application form (D201), fully completed
- Two recent identical Passport Type Photographs (signed on back)
- Current or most recent provisional licence. If lost, form D8a to be completed

- The second provisional licence is valid for two years
- You may drive, unaccompanied by a qualified driver, if you hold a second provisional licence in category B
Third or subsequent Provisional Licence
Licence application must be accompanied by:- Application form D201, fully completed
- Two recent identical passport type photographs (signed on back)
- Current or most recent provisional licence. If lost, form D8a to be completed.

Important Note:
The following forms:- D.401, Application for Issue Or Renewal Of Driving Licence,
- D.800, Application for Duplicate Driving Licence,
- D.201, Application for Provisional Driving Licence,
cannot be downloaded from the website at this time due to a specific signature requirement which has to be affixed to your licence.They are available at our offices, City Library and Garda stations.
