Duplicate Provisional or Full Driving Licence

If your licence has been lost, destroyed or stolen and you wish to apply for a duplicate licence, your application must be accompanied by:

• Application form (D800), fully completed and also signed, dated and stamped at your local Garda Station.
• Two recent identical passport type photographs (signed at back by applicant)
• Fee of €5.00

Important Note:

• D.401, Application for Issue Or Renewal Of Driving Licence,
• D.800, Application for Duplicate Driving Licence,
• D.201, Application for Provisional Driving Licence,

cannot be downloaded from the website at this time due to a specific signature requirement which has to be affixed to your licence.They are available at our offices, City Library and Garda stations.
